FORMS & DOCUMENTS

                                               PLEASE, REMEMBER:
To have your property listed, you must fill out and submit ALL forms
listed below in the Section I.


SECTION I. Forms you need to fill out
to have your property listed on the MLS

The following forms can be opened with Adobe (files are in .PDF format). If you do not have Adobe, click hereto download it.


1. Agreement of Service (with Service Description)
This is our contract by means of which you order and we provide you this Service. Please, review the document and print it out. After that, fill in the desired end date and initial immediately next to it (p.3), amount of commission you choose to offer to a buyer's agent and initial immediately next to it (p.8), any additional conditions we may have agreed on, sign it, date it and send to us by mail, fax or email.


2. Property Profile (MLS Data Entry Sheet) - Residential
This form is for collection of information about your property that will be displayed on the MLS. Use this form only if you are selling a single family home or a site condo.

IF YOU ARE SELLING A CONDOMINIUM, please, fill out THIS FORM.

Please, remember:
 
 a) it is your responsibility to provide information about your property and make sure this information is accurate;
 
b) what you give us will be placed on the MLS; respectively, if you miss something, it will not be on the MLS; we will not be asking you to add or amend anything later;

c) if you prefer to submit your own photos (up to 6 in the Realcomp MLS, and up to 9 in the MiRealSource MLS) of your property to be displayed on the MLS, you have to email them to us within FIVE days from the date of signing the Agreement of Service. We strongly recommend that you email us these photos the day your property goes on the MLS, otherwise it will be just a listing ticket with "no photo available" box on the top of it.


After you fill out the form, please do not forget to sign and date it.


3. NEW FORM - Listing Services Disclosure Addendum
As of May 14, 2007, MiRealSource MLS requires brokers and sellers to sign a new form - the Listing Services Disclosure Addendum - in all cases when a broker takes a non-Exclusive Right To Sell/Full Service listing.

HOW TO FILL OUT THIS FORM:
  • Write your property full postal address in the 2nd line of the form;
  • In Paragraph 1, line 1 check "NO"; line 2 - "YES" (only if you have a sign); and line 3 - type of your sign;
  • In Paragraph 2, check "YES" and then write the end date of your home's listing;
  • In Paragraph 3, check "NO", then  the end date of your home's listing; in the following sections of this Paragraph, check "NO" in every sub-section and write your name, phone number, title (e.g. - HOMEOWNER), and the end date of the listing period IN EVERY SINGLE LINE afterwards. Alternatively, you may write in the name, phone number and title of the person who will handle the mentioned activities for you;
  • Sign and date the Addendum

 4. Disclosure Regarding Real Estate Agency Relationship
This form establishes Real Estate Agency Relationship between you and HAVEN Realtors. Please, check "Transaction Coordinator" box - this is the only form of real estate agency relationship we will establish at this moment. On the bottom, please, circle the word "Seller", then sign and date this form.



SECTION II. Forms you will need to fill out
and furnish to the buyer of your home

1. Sellers Disclosure Statement
By law, you have to furnish buyer for your property with the Seller's Disclosure Statement prior to closing. You do not need to provide buyer with this form if you are a builder selling a brand new never lived in spec home, or a lender who has obtained the property in the result of foreclosure.

2. Lead-based Paint Disclosure
By law, you have to furnish buyer for your property with the Lead-based Paint Disclosure if your home was built before January 1, 1978.
